New Student Enrollment, Packet Pickup, Walk Through Registration, Freshman Orientation
2013-2014 CVHS STUDENT ANNUAL REGISTRATION INFORMATION
Following is important information regarding, Packet Pickup, Summer Walk-Through Registration, Freshman Orientation and New Student Enrollment for the 2013-2014 school year.
Volunteers needed to hand out packets! Please email the date(s) you are available and your phone number to cvhsreg@gmail.com.
If your student attends one of our feeder middle schools they are automatically in our database and they just need to pick up our Summer Walk-Through Registration packet in August and participate in the walk-through registration. Please read through all the information and check back periodically for updates.
PACKET PICKUP DAYS AND NEWSPAPER DRIVE
Tuesday, Aug. 6, 2013 9am-12noon
Wednesday, Aug. 7, 2013 4pm-7pm
Thursday, Aug. 8, 2013 9am-12noon
Students and/or parents can drive thru the upper parking lot and drop off newspapers and then pick up the registration packet in the lower parking lot.
Read your packet thoroughly! Re-Registration has become more streamlined! The packet includes all the forms necessary to start school and much more.
SUMMER WALK-THROUGH REGISTRATION FOR ENROLLED STUDENTS
During the registration process students will be able to do the following and more:
* Turn in required forms to start school
* Receive list of classes
* Purchase ASB cards, PE uniforms, spirit items and parking permits
* Have ID photos and yearbook photos taken

Following is information for students new to the Capistrano Unified School District (from out of state, from private schools, or moved into our boundary). Please call CVHS at (949)364-6100 to confirm your address and that we are your home school. Once we have confirmed your address please go to cvhs.com and New Online Student Enrollment or click on the link below to begin the enrollment process. When we receive your online enrollment information we will contact you and continue the process.

When you come to the school to finish the enrollment process there may be other paperwork to complete. You MUST bring the following to complete the enrollment process:
Please bring the following:
- 2 forms of Proof of Residence (must be current electric, gas or water/sewer bill AND mortgage, lease or rental agreement)
- Immunization Records
- Transcript
- Birth Certificate (If student has not previously attended school in California)
At the New Student Enrollment you will complete the enrollment process, and briefly meet with an Advisor to determine your students schedule for the Fall Semester.
Matriculation Tests
Reading
If your student is an incoming 9th grader they will be taking a reading placement test at 8:30am on the date above. The test will take approximately 1/2 hour depending on the student. Please expect the entire enrollment process to take at least 1 ½-2 hours. If your student is not available to take the test on the date above please check back for further instructions as dates become available.

You may call to verify you are in our attendance boundary and complete the online enrollment processes before the above date to pre-enroll. YOU STILL MUST ATTEND THE DATE ABOVE to matriculate, speak with an Academic Advisor and schedule classes. Your enrollment will not be complete if you do not attend New Student Enrollment. After you are enrolled you will be able to pick up your Registration Packet in August and your student may attend the Summer Walk-Through Registration. PROOF OF RESIDENCY (CUSD School Board Policy cf.5111. Admission)

The enrolling student must be living with a blood relative or guardian who lives within the Capistrano Valley High School attendance area. The relative/guardian must submit two documents for verification of residency(must be current electric, gas or water/sewer bill AND mortgage, lease or rental agreement). If the student and parent reside with someone else they must have a driver’s license with the new address. A bank statement or other bill with their name and address on it will suffice as well. |

BIRTH CERTIFICATE |
A copy of the student’s birth certificate is required if the student has never been enrolled at a public school in California.

IMMUNIZATION RECORD |
Students will not be admitted without proper documentation of immunization. A copy of the “Yellow” Health Record or other Official Health Record (school or doctor) must be provided to the school. For the 2011-12 school year only, all students entering 7th through 12th grades will need proof of an adolescent whooping cough booster shot (commonly called the “Tdap” vaccine) before starting school. |

UNOFFICIAL TRANSCRIPT OR REPORT CARDS |
An unofficial transcript is necessary to ensure the student is placed in the appropriate classes to meet CUSD graduation requirements. Please provide this document. Any student transferring mid-semester will need to bring “in progress withdrawal grades” from their previous school. After a student has enrolled at Capistrano Valley High School, the guidance office will request the official records from the previous school. |

- If you are assuming responsibility for a child with the permission of that child's parents, you will need to complete a Caregiver's Authorization Affidavit. Forms are available at the school from the Registrar. The form must be completed prior to the registration meeting with the academic advisor.
- If your child needs Special Education, please bring a copy of the latest I.E.P. (Individualized Education Plan)
- If you are requesting Honors placement, please bring certification and placement recommendations from the previous school.
- If English is the student's second language, preliminary assessment will be done. Further assessment may be needed.
